Form SSA-7162
If you live in the Philippines and collect U.S. Social Security benefits, there's one form that can stop your payments cold if you miss it: Form SSA-7162. Today I'll show you exactly how to submit it — including a new option that lets you skip the mail entirely.
WHAT IS IT
Form SSA-7162, officially called the "Report to the United States Social Security Administration," is sometimes nicknamed the "proof of life" form. The SSA sends it periodically to beneficiaries living abroad to confirm you're still alive, your address is correct, and nothing has changed that affects your eligibility. If you don't return it within the deadline on the form, your payments can be suspended.
What If I don't get form in the mail
Didn't get the form in the mail? You don't have to wait. You can download a blank copy directly from the official Social Security website and fill it out yourself.

Unless you have a duplex printer the two pages will be separate. I marked both pages, 1 of 2 and 2 of 2. I also wrote my name and date of birth and SSN on second page so that if they are separated it can be matched to page one. Be sure to initial each page in the margin.
After Completing the form 7162
Mail it to the embassy: Whether you received the form in the mail, or downloaded and completed the form, sign it and send the original ( not a scan, fax, or email) to the Social Security Administration's Federal Benefits Unit at the U.S. Embassy in Manila. This is the correct address:

A quick tip: send it through a courier service with tracking, since regular mail can be unreliable.
I think it is much easier to just send the form to the FBU in Manila, but if you want to, you can do it via telephone.
Here’s the part most people don't know. As of January 2025, Social Security officially allows you to complete this form by phone instead of mailing anything. It's called "attestation," or signature proxy — you verbally confirm your information and swear to its accuracy, and an authorized Federal Benefits Unit staff member documents that on your behalf. It carries the same legal weight as your signature.
This option is only available through your local Federal Benefits Unit for Philippine residents, that's FBU Manila. To use it, email FBU.Manila@ssa.gov to request an appointment, or call them directly at +63-2-5301-6200, available Tuesdays and Thursdays, 8:00 to 11:00 AM Manila time.
